Just the paper sleeve. There is a staple on the upper left corner.  I do great combined shipping !

*********** I just came across an old record price guide. This record was listed. It lists it as $500 bucks in mint shape. Mine is in really nice shape probably because it spent most of its life in a dresser drawer. The only real flaw is the staple in the corner. Just wanted to let you all know. The price guide doesn't really say whether the $500 is with the record, or just for the picture sleeve. The book is called "Goldmine Price Guide to 45rpm Records".
