There are 2 vinyl records here.

The first LP (pictures 1, 2 & 3) -  TPRS 1895-1K - rejected / 1896-1K - OK.  I think the side one was rejected, and needed to be redone.  And the second LP (pictures 4 & 5) TPRS 1895 - 2K - OK.  The second LP only has side one - the other side contain test tone. 

There is no cover.
