Original inner sleeve.

Record: Side 1 - M-                Side 2 - Mostly EX+ with cut 3 having a vinyl defect 1/2 way thru that sticks then pops .                Side 3 - M-/EX+                Side 4 - M-/EX+

Opened records cleaned on VPI machine with Disc Doctor.
