Per G45Central.Com there have been just two known copies until now. U.S.A. 856 thought to have been an unissued # by major U.S.A. label collector. G45 List #176 - Comped on: Teenage Shutdown (TS-6613) & 2131 South Michigan Avenue: 60's Garage & Psychedelia From USA and Destination Records (LP 5245) The labels on this copy appear cleaner than the copy photographed and used for the meticulous 2131 South Michigan Avenue set. They are perfectly clean.
Condition: This record was evaluated independently by two prominent Chicago record stores. Both gave it a visual grade of VG+/M-. There is a small manufacturing "bubble" in the dead-wax of side A, and another, extremely tiny mid-track. There is another on the B side towards the end of the track. Both sides tested at store on a Tec 12 at both 1.5 and 3.5 Grams with no deterioration in sound quality from bubbles or superficial scuffing. This test was most likely the first or second play of this record, any signs of wear in the past having resulted from sleeve/handling. True Garage Fuzz masterpiece in all its glory. Sleeve is generic brown as-found, assuming these DJ Copies weren't placed in consumer USA sleeves. (Grading metric used by store below)
This copy was found among other 45's that were all DJ/Radio/Promos of "A Bee-Anne Production" records; Quill Records, Kapp, and USA 45's. All were in VG+/M- condition in original sleeves. The Great Society hailed from Villa Park IL. G45 Central publishes USA 856 is "by far, the rarest USA label 45 and only 2 known copies". "Lack of legend status is the only thing holding this one back from higher G45 rating", however this record has rose to prominence in recent years. As far as I can tell the other 2 known copies are DJ Copies as well. Please message with any questions, interest, for additional information.

Still-Sealed Mint (SS): When someone says something’s “Mint condition,” this is what they’re talking about. The album is sealed in its original packaging and has never been played.
Mint (M-): This is the highest grade for records that have been played. The album is in excellent condition and shows no visible signs of wear or damage that will affect sound quality.
VG+: While not flawless, the condition is very good, showing only minimal signs of use. Any visible blemishes should have minimal affect on play quality, though light surface noise maybe present at very quiet portions or between tracks. This is a record that should satisfy all but the most strict of collectors. There may be slight ring or cover wear, or minor writing on the cover.
VG: Vinyl may have scratches and/or other visual blemishes, but should play through without skipping or repeating. Light to moderate surface noise and/or crackle may be present, however not overly distracting. Covers may have moderate ring and/or edge wear, cut-outs / hole punches, or a moderate seam splits.
VG- and below: Let’s be honest: sound quality is low and the cover may be in very poor shape. We consider any item graded at or below VG- as a LOW GRADE item.
