Visual-- Cover -- VG condition with a 2 inch piece of clear tape on the center of the spine, see picture. Label-- EXCELLENT condition. (The blurry pictures do not do the record justice. It looks like the day it was taken out of the cover.  Vinyl-  EXCELLENT condition.  Audio-- Both sides are in EXCELLENT condition. This very rare EP is really beautiful.
