The cover is very good (a small, perhaps 1 cm, hole was punched through the upper right hand corner. This is visible in the photograph)
I mean, it's not a big deal or anything, but it has the original inner sleeve.

Yes, it does have the gatefold cover, and, as I understand it, the US cover was made out of better cardboard than the UK cover. (whatever that means)
Yes, it has the original light blue island inner sleeve.
Yes, this is the original US pink rim label.
The matrix numbers etched in the vinyl are
smas- 1- 9318-H4
smas- 2- 9318-H4
