This is the 1st single by Nirvana, but not the very first edition.  The very first edition was limited to 1,000 and hand numbered, then they made another 200 which weren't numbered, but had a red mark where the number should have been.  This record says "Left Over" Edition #152/250 on the back cover.  It is a one sheet foldover cover, as was the original, but is a little darker from what I can find online as a couple names are a bit hard to read.   
Looking it up online, on the Nirvana discography site, someone wrote a while ago that he thinks (but wasn't 100% sure) that he got one of these directly from Subpop.  That would make sense with calling it a leftover edition... maybe they had some more, but I think it's more likely that they had some leftover orders to fill and did another pressing.  The numbers were stamped... the number is pretty light on this one.  They probably learned from the first batch that it was too much work to do it by hand. 
There's writing in the Matrix runout saying "Why don't you trade those guitars for shovels"  The original pressings also had that in the Matrix.  Later counterfeit copies do not say that, and most have anything in the runout scratched out.  There are some other ones around that say 'Left Over Edition' but are not numbered, and those do not have the matrix etching the same.   
Between the Matrix etching, the numbering and the story of someone getting it from Subpop, I think it's likely this was put out by them, but I can not guarantee that.  
I got this through my store which closed in 1999.  I don't remember when, but I do know I had it set aside for myself for a long time before we closed our doors.  So, at the very least, it's over 17 years old, and quite possibly from 1988 or 89.  I'm not going to guarantee that it is a Subpop pressing, I'm only giving as much info as I can to help you decide.  I've seen the early counterfeits sell for $200-300.  The original not leftover additions go for over $2K.
