Housed in a tri-fold cover and includes a small-format lyric booklet.
Label misprint on side C: "Psychedelic Pill (Alternate Mix)". Tracks B2 and C1 are both labeled "Psychedelic Pill (Alternate Mix)", whereas only track B2 matches the alternate mix included on CD versions of the album. The correct title for C1 is "Psychedelic Pill".
Runouts are listed starting with the etched part; the stamped part starts with "-260" on all sides.
