Super rare, limited edition Less Than Jake Pizza Box 7". Only 1000 copies made -- this one is #587/1000. 
I played this record 3-4 times back in the 90s, and it has been sitting at my parents' house ever since. I can't find a single mark or scratch on the record, and the pizza box has held up well.
