This record is a radio station copy and is in near mint condition. The cover has a slight tear visible on the reverse corner..
I purchased this record here in Toronto at radio station CJRT Jazz 91.1 FM after they went digital and held a record sale in 2016. 
The inscription reads (see photo) "To My friend Joe with admiration and best wishes - John Hammond" There is also written in ink "Joe Lewis" and "1969"
From the web: "Joe Lewis, joined CJRT-FM part-time in 1965 to host a four-hour programme, "Folk Music and Folkways" which stayed on the air for 31 years"
