Overall this single is in excellent condition. It is on the London label,  has the catalogue number HLZ 10059 and on the A side is the wording London American Recordings Demonstration Sample Not For Sale. It was manufactured in the United Kingdom. 
The record is in excellent condition with faint spindle mark.  The matrix numbers are 45-MSZ-7569-1C and 45-MSZ-7570-1C.
The record comes in a plain white paper sleeve.
