Matrix says:  PL-1015 A and possibly P/WDM and PL-1015 B and possibly FWG  dm, these parts are hard to read.
Sleeve is in Great Condition, with minor wear on each of the 3 spines, but still looks Great and almost New. There is a small dent in the opening on the back side of the sleeve.
Record looks Great, fairly new, and glossy. One tiny dent or scratch in the groove. Some minor dust.
