Fantasy Records 12-inch 33 RPM LP # 8082 has three titles. The record is labeled "Fantasy Presents Mulligan-Desmond." The front cover title is "Gerry Mulligan, featuring Chet Baker, Chico Hamilton, also Paul Desmond, Barney Kessel, David van Kreidt." And the back cover title is "Paul Desmond, featuring Barney Kessel, Dick Collins, David van Kreidt, Joe Dodge, Bob Bates, also Gerry Mulligan, Chet Baker, Chico Hamilton." Pressed on blue vinyl, it is copyright dated 1955 on the front cover. On later issues, I believe side one was labeled as the Gerry Mulligan Quartet, and side one as the Paul Desmond Quintet. Although the labels do not specify mono or stereo, there is a "Full Radial Stereo" sticker on the front cover.
The record looks to be only in about VG condition, still nice and shiny, but with visible surface scuffs and scratches. I've played both sides on my Audio-Technica turntable, with no skips or jumps, but some audible background noise, including some clicks and pops and ticking. Because of variations in tracking ability, filtering technology and needle weights, I can offer no guarantees as to how well it will play on your equipment. 
Apart from the heavy dampstaining you can see in the pictures, the cover is in very good and functional condition, but with a price sticker on the back and the top seam starting to split at the open end, otherwise just typical light surface soiling and edge wear. It comes with a plain white inner sleeve.
