Grading: I use the following grades: NM, EX, VG+, VG, G, F, P. Sometimes, if the record is very borderline, I will use the lower grade, and add a (+) in parentheses to indicate that it is an unusually nice example of this grade category.

I grade conservatively, following the Goldmine grading system standard, with the the common addition of EX as a grade to represent records falling between VG+ and NM.
