4 track 45 rpm picture jacket
IMPERIAL records (EP 151 on the jacket, as well as IMP-151
and the labels state EP 201 b/w EP 202)
the Vinyl is VISUALLY Graded: 
Vinyl looks VG+ to VG++ on both sides;

Jacket looks VG++ on the front, VG+ on the back:
light water staining at 2 of the 4 edges, but it's barely visible on the front, which has a mostly
very nice looking, semi-glossy laminate, and the spine and seams are intact!
