**WITH UNUSED FAN CLUB INSERT**
This rare initial first pressing copy comes complete with its original Beatles Fan Club special offer application form valid until July 31 1968
The disc is a 1968 first pressing on the black & silver Parlophone label R 5675
Both labels carry the 'Sold in UK.. statement and 'Gramophone Co Ltd...' rims.
Housed in its original, correct type 'Parlophone' company sleeve with the 7/- to 50/- EMI Record token price on the reverse.
Matrix numbers: 7XCE 18438-3 & 7XCE 18439-2
Stamper numbers: 4/OP & 3/RP
Insert: EXCELLENT
This super clean and un-yellowed example is printed on matt paper displaying just some light storagewear.
The perforation unworn and is 100% intact and with no marks or writing anywhere.
The green 'Parlophone' sleeve has the title 'Lady Madonna' written in ballpoint pen in the front upper right corner.
