NOTE: on LP-2 [LAFMS-11], cover artist Don Van Vliet (aka CAPTAIN BEEFHEART) was mis'credited as "Donald" so, as you'll in pic #2, someone had to cut out the "ald" from "Don" with an exacto knife on all copies of the record. There's a neat & tiny little rectangle removed from the top layer of paper & the outer plastic... Still MINTy.
